MADI interface card missing channel audio?

Summary of the defect:

The MADI receive buffering is skipping a channel (randomly). This was observed in the analysis of the receive buffer address generation and is due to an error when synchronizing from a fast clock domain to slow domain.

Reason for the defect:

The problem was identified with the MADI locking to external word clock for transmission. This was due to a signal integrity issue with the word clock input.

The solution:

Update to the FPGA firmware for the MADI card, fixed in version V6 (July 2019)
