Clear-Com Concert License request form Dear Customer, Thank you for purchasing Clear-Com Concert. In order to enable yourConcert User license, you need to load a Concert License Key onto your server. Please enter the following information: Date:                                       Customer contact Name:       Customer email address:       Clear-Com order number:     Concert SERIAL number:      
 To get the Concert serial numberGo to Concert server > Admin > licence


If ordering for multiple Concert Servers please send a separate requestform for each Server. Please email your completed form to the following email address: CCConcertLicense@clearcom.com
