Matrix: What are the port pinouts on a Matrix +2 frame MTX-100 card?

On the rear panel of a Matrix +2 frame (MCF-10 / 25 / 50 or 100) the MTX-100 ports are dB-15M connectors for each port.  The port wiring pinouts are shown below, the cabling used can be either 3 pair or 4 pair twisted pairs.
